A wounded woman named Sivagami (Ramya Krishnan), carrying a baby in her arms and wading through a river under a massive waterfall, is being chased by a few soldiers. After escaping from them, with nowhere to go and in order to save the child, whom she calls Mahendra Baahubali, she decides to drown herself in the water while holding the child above it. She proclaims she will not let anything happen to the child come what may. At sunrise, a few villagers spot the child stranded alive in the middle of a river. As the infant is saved, Sivagami points to the top of the mountains and waterfalls (the direction of Mahishmati Empire). Sanga (Rohini) and her husband rescue the infant and raise him as their own.
The boy grows up to be the mighty Shivudu (Prabhas), who aspires to climb the waterfall to see what lies up there. His curiosity irks his mother as it could be dangerous. After many failed attempts, he finally makes it to the top. Looking for the owner (Tamannaahas Avantika) of a mysterious mask which fell in his lap, he discovers that unlike the lady of his dreams, she is a rebellious warrior fighting against the tyrannical king Bhallala Deva (Rana Daggubati), who has captured queen Devasena (Anushka Shetty) and has been torturing her for the past 25 years. Her group indulges in guerilla warfare against the king to rescue the queen.
While initially she doubts Shivudu's intentions, on seeing her mask with him, she realises he is innocent. The two fall in love and Shivudu pledges to help her in her mission. He sneaks into Bhallala Deva's Mahishmati Empire to rescue Devasena but gets caught while escaping with Devasena. The king's loyal guard Kattappa (Sathyaraj) attacks Shivudu but given his strength, and after seeing his face realises Shivudu is the late king Amarendra Bahubaali's (Prabhas) son Mahendra and deserving heir to the Mahishmati Empire.
The story then goes into a flashback showing the story of cousins Amarendra Baahubali and Bhallala Deva. Though Bhallaladeva's father Bijjala Deva (Nassar) is the elder brother, Shivudu's grandfather is crowned as the king since he is more worthy. Bijjala resents this decision as he thinks this happened because of his physical handicap. After his brother (the king) dies, Bajjala wishes his son Bhallala to be the new emperor but the late king's wife before dying gives birth to Amarendra Baahubali, another heir to the Empire.
Bijjala's wife Sivagami is feisty, fair and impartial. She raises both the children Bhallaladeva and Baahubali as her own and proclaims that whoever proves to be more deserving will be crowned as the new king. Later, as the Kalakeyas are preparing make war against Mahismathi, she agrees to announce the one who kills the chief of the Kalakeyas as the King. Bijjala and Bhallala's tactics fail as after witnessing the battle, Sivagami announces Baahubali to be the new king, even though it is Bhallala Deva that kills the cheif, as he protected his own people while making war. Bhallala Deva is appointed as the Commander-in-chief of Mahishmati's military.
While sharing this truth, Kattappa is interrupted by Shivudu who asks that if his father was so strong who killed him. Kattappa, with tears in his eyes, reveals himself as the one who betrayed Amarendra by killing him.

Amarendra Baahubali (Prabhas), Bhallala Deva (Rana) both love Devasena (Anushka). Devasena likes Amarendra Baahubali. Baahubali’s love not accept his mother. So, Baahubali leaves the kingdom for the sake of his love. Devasena and Baahubali spend happy life together. Meanwhile, Bhallala Deva treats the people of Mahishmati as slaves and people shows their discomfort on Bhallala Deva. Kalakeya brother (Charan Deep) announces war on Mahishmati. Baahubali comes to know the fact that Bhallala Deva is losing his power, Baahubali enters in the war and won the kingdom.
Unfortunately Baahubali loses his life as Bhallala Deva and Bijjaladeva Deva join together with Kattapa. At the same time Devasena gives birth to a baby boy. Sivagami (Ramya Krishna) saves the baby boy and he grows up as a tribal boy. With this, the flashback episode completes. Now, Shivudu (Prabhas) along with Avanthika (Tamanna) fights with Bhallala Deva and finally, they kill him and Shivudu gains his kingdom.
